COVID-19 vaccine exemptions

Michigan State University is requiring students, faculty and staff to be vaccinated against COVID-19, with limited medical and religious exemptions for all, as well as exemptions for students if they are taking all online classes and will not be on property owned or governed by MSU for any reason during the fall 2021 semester. Even those seeking an exemption MUST fill out the vaccine verification form.

All individuals who requested a medical or religious exemption prior to the deadline must enroll in the Early Detection Program while they wait for their exemption requests to be either approved or denied (students seeking an online course only exemption should not be coming to campus at all). If approved, individuals with a medical or religious exemption must remain enrolled in EDP. For those not approved, they must either receive a COVID-19 vaccine or cease coming to any MSU-owned or -governed property. Those in violation will face discipline, including and up to expulsion or termination.

Tips for filling out your form correctly

  • Include a verified electronic or penned signature on your exemption form
  • Submit forms as non-password protected attachments only (links will not be accepted)
  • Include all supporting documentation relevant to exemption reason
  • Include your APID/ZPID
  • Please send requests from MSU email accounts; if not, please confirm NetID in the body of the email

Appeal process

Faculty, students, and staff who applied for, but were denied, an exemption from the mandatory vaccine requirement may appeal the denial determination by following the appeal process.
